The Progressive Housing Society (PHS) and Burnaby PCN have partnered in an innovative initiative that aims to transform healthcare delivery for vulnerable populations in Burnaby, BC. At the heart of this initiative is the search for a dedicated Nurse Practitioner to join the onsite clinic and allied health team at PHS.

Bridging the Gap in Healthcare

People facing mental health challenges, addictions, and homelessness encounter significant barriers when trying to access healthcare. These barriers include lack of stable housing, stigma, limited access to transportation, and difficulty navigating complex healthcare systems. These challenges often result in fragmented care and unmet health needs, exacerbating their conditions and hindering recovery.

Recognizing these critical issues, the collaboration between the Progressive Housing Society (PHS) and Burnaby PCN aims to bridge this gap by providing holistic and accessible healthcare services. This initiative, part of the Primary Care Network, is dedicated to delivering compassionate and coordinated care to individuals experiencing homelessness and those in transitional housing. The populations served often face severe and persistent mental illnesses alongside other medical complexities, making this a critical program in supporting these individuals through the initial stages of primary care.

A Unique Opportunity for Nurse Practitioners

This collaboration between PHS and Burnaby PCN offers both full-time and part-time positions for dedicated Nurse Practitioners who are passionate about making a difference in the lives of vulnerable individuals. These roles provide a flexible schedule of 4-5 days per week on-site, with the option for partial virtual work when clinically appropriate. The roles come with autonomy in scheduling and a supportive team-based environment. Reduced attachment targets are implemented to ensure quality care for the high-needs population served.

The responsibilities of the Nurse Practitioner include comprehensive patient assessment, diagnosing diseases and conditions, initiating treatments, and prescribing medications in accordance with regulatory standards and policies. This role involves close collaboration with PHS staff and other healthcare professionals to deliver coordinated care. Additionally, the Nurse Practitioner will participate in policy-making activities, population health initiatives, and continuous professional development to improve health outcomes at both individual and community levels.
